FROM THE NURSE: The Department of Health requires all immunizations be up-to-date before a child attends school. If immunizations are not current, an “in progress” card from the physician as well as updated immunization records must be submitted prior to August 25. The State of Missouri requires all Seniors and 8th grade students receive a meningococcal (Menactra) vaccine, and all 8th grade students receive a T-dap immunization. If your child has already received either vaccination, please submit documentation to Fatima at Open House, or prior to August 25. If your child has not received either vaccination, or if you are unsure of your child’s immunization status, please contact Fatima at 573-455-2375 ext. 1015 or 1014
REGISTRATION for RETURNING STUDENTS K-12 will take place Mon Aug 4 and Tues Aug 5, 8am-3pm each day. Mandatory forms will be distributed and must be completed and returned PRIOR to the start of school Aug 25. A parent/guardian must also complete the online portion of the registration process. Please log in to your parent account to access the OLR. If you have not yet successfully set your parent account up, you will need to contact the appropriate office to receive instructions. All JH/HS students and a parent/guardian are required to sign a Technology Usage Agreement and pay a $15 non-refundable deposit to cover insurance prior to the Chromebook being issued. The updated Student Handbooks will be available on our website. You are strongly encouraged to read it in its entirety prior to the start of school. An acknowledgement that you have read the handbook must be signed and submitted prior to 8:00 a.m. Mon, Aug 25. ALL families are now REQUIRED to submit a document as proof of residency. The document MUST be in your name at the address you have reported as your residence (rental agreement, utility bill, etc.).
NEW STUDENTS: K-12 students who reside within the Fatima school district who did not attend Fatima last year and have not yet registered for this year should begin the mandatory registration process online. A parent/guardian must contact the appropriate office to notify of an impending enrollment, and to receive online enrollment application instructions. A parent and the student will then need to report to Fatima Thurs Aug 7 or Fri Aug 8 between 8:00 AM – 2:00 PM to meet with the appropriate guidance counselor to complete the transfer process.
GENERAL INFORMATION: A deposit should be made into lunch accounts prior to the start of school. Picture Day for K-12 is Wed, Aug 27 (senior photos will be taken for the class composite). All students must have a photo taken for school lunch ID’s. Retake day for K-12 will be Wed, Nov 5.
OPEN HOUSE: Open House for Elem, JH, and HS will be Thurs Aug 21, 4pm-7pm. There will be a Ribbon Cutting ceremony for the new High School at 5:00. Fatima will be open to all students/families – Chromebooks will be issued to 7th-12th grade students at Open House. Be sure you have the signed the Tech Usage Agreement and that you have $15 for the deposit.
FRESHMAN ORIENTATION: Freshman Orientation will be Thurs, Aug 14, 6:00 PM. All incoming freshmen are strongly encouraged to attend.
SCHOOL BREAKFAST / LUNCH PROGRAM: Aug 25 is a full day of school and breakfast/lunch will be served. Applications for free/reduced school meals will be available at Open House. If your child qualifies for F/R meals, please know that you are responsible for all charges incurred prior to your application being approved. It is strongly recommended that applications for F/R Meals are returned to school PRIOR to Aug 25. It is also strongly recommended that all families complete and return this form as all qualifying students and the school greatly benefit from this program. All forms are processed with strict confidentiality.
ACTIVITY FEE and PHYSICALS: Students will be assessed a $20 PER ACTIVITY fee to participate in clubs/sports ($100 family cap). All students participating in a school sponsored BAND or SPORTS ACTIVITY must have an active Privit account and completed physical prior to the start of practice. Physical forms can be obtained in the HS office. All students participating in a school activity must be in the drug testing pool or they are ineligible to participate in the activity. The consent for drug testing form will be available at Open House. Fatima requires a sports physical on a yearly basis.
