MSHSAA has approved sports physicals for two years, however, Fatima will continue to require a new physical for each school year. We are sorry for any confusion and inconvenience this may have caused. Parents will also need to update insurance information, participation forms, drug consent form and other forms yearly.

PRIVIT is a software program which will allow coaches and administrators to access student athletes' physicals and emergency contact information on their mobile devices. Click below for directions to set up your student's account and fill out the necessary forms. If you have already turned in a copy of the physical to the school, you will still need to set up an account and fill out the forms. Once you have done this, contact Lisa Neuner and she can upload the dr. physical to your account. If you have any questions, please feel free to contact Lisa at ext. 2002.
